Clinic cards provide the school with emergency contact information as well as medical information such as any chronic condition (allergies, asthma, etc.) that your child may suffer from. You will be asked to complete a clinic card for your child during Meet and Greet at the beginning of the school year. If you did not complete one, please print a clinic card from the link below, complete, sign and return it to school with your child as soon as possible.
Parents are expected to keep emergency contact numbers up to date. If the information for yourself or your emergency contacts changes, please notify the office/school nurse immediately. Parents should also inform their children's classroom teachers of all known allergies and sensitivities. In addition, it is important to inform the School Nurse to discuss specific needs and medical interventions.
