Medications should be given at home whenever possible. For instance, medication prescribed for three times a day can usually be given before school, after school and at bedtime. If your child needs to take medication during school hours, please bring the medication to the clinic/front office and complete and sign the permission form. Due West is required to follow the guidelines established by CCSD when administering medication to students while at school. These guidelines apply to both prescription and over the counter medication. Below are the guidelines, in part, that CCSD schools are required to follow:
- Written Permission of the parent/guardian is required for the administration of all medications.
- Medications must be brought to the office/clinic by the parent/guardian.
- Medications must be in the original labeled container. Pharmacists may provide two labeled bottles for this purpose. Medications sent in an unlabeled container or baggie will not be given. If your child takes daily medication, please send an extra bottle to be used for field trips and the After School Program. (Do not need to send an extra inhaler or Epipen for field trips or ASP if one is already kept in clinic.)
- The parent/guardian must inform the school of any medication changes. New medication or new doses will not be given unless a new form is completed.
- Unused medication will be disposed of unless picked up within one week after medication is discontinued. If medication is given throughout the school year, medication will be disposed of according to the medication Rule Section IX.
Prescription Medication:
- A prescription from a legal prescriber is required for prescription medication. The pharmacy label may be used as the health care provider’s written directions.
- Students who are new to the District and bring in prescription medications ordered by legal prescribers from states other than Georgia will be allowed thirty days to obtain new prescriptions from a health care provider licensed to prescribe medication Georgia.
- Medication samples must have a written prescription or a legal prescriber’s written order or note bearing the student’s name.
- A student, with the signed approval of their Legal Prescriber and parent/guardian, may carry and self-administer the following prescription medications:
• Asthma medication
• Epinephrine auto injector
• Diabetic medication

The Due West Clinic has a nebulizer machine for student use during the school day. Parents/guardians need to provide the facemask, tubing, medication and appropriate authorization forms.
Over-the-Counter Medication (OTC):
- May not be administered in doses that exceed established amounts for age or weight as printed on the manufacturer’s label. Continued consecutive administration of the OTC medication after ten days will require a legal prescriber’s written order.
- Medications, including over-the-counter medications (i.e. Tylenol, Advil, antibiotic ointments, calamine lotion, cough drops, etc.) will not be supplied by the school or District employees.
- Changes in dosage or time of assistance with the administration of medication are only permitted with written authorization from:
• Over-the-counter: The parent/guardian;
• Prescription: Both the parent/guardian and the Legal Prescriber.

Clinic Cards
Clinic cards provide the school with emergency contact information as well as medical information such as any chronic condition (allergies, asthma, etc.) that your child may suffer from. You will be asked to complete a clinic card for your child during Meet and Greet at the beginning of the school year. If you did not complete one, please print a clinic card from the link below, complete, sign and return it to school with your child as soon as possible.
Parents are expected to keep emergency contact numbers up to date. If the information for yourself or your emergency contacts changes, please notify the office/school nurse immediately. Parents should also inform their children's classroom teachers of all known allergies and sensitivities. In addition, it is important to inform the School Nurse to discuss specific needs and medical interventions.
